This “Terms & Conditions” shall form part of the agreement between you (Member) and Pros-Apac Corp (PAC) in accordance with Prostige Platinum Rewards (PPR) Program. Use of the Prostige Platinum Card (PPC) is conditioned on your acceptance without variation of the Terms & Conditions set out here in which Pros-Apac may change from time to time at its sole discretion with or without notice. Your signature herein and your use of this Card when purchasing constitute your express consent to all the Terms &Conditions as stated herein.
1. All valued customers of Pros-Apac Corp (Dentists, Dental Center, Dental Laboratory Technicians, Dental Laboratories), are qualified to avail of the Card Program upon filling up the Prostige Platinum Card Application Form, except Dental Students & Universities.
2. Card doesn’t have expiration. However, Earned Points are only valid for use before the end of January of the following year.
3. In the course of the membership, PAC may request to provide and/or update Member’s personal data. The Member agrees and hereby gives consent to PAC to collect, store, access, share, process, his/her personal data including any updates or changes thereto, whether manually or electronically, for the purposes of the PAC membership and program, including any renewal thereof, as well as of conducting marketing and customer profiling activities in connection with PAC’s services and related products, and issuing or sending related notices, updates, service or administrative announcements.
4. Lost or damaged Card must be reported immediately and may be replaced by submitting a completely filled-out Application Form & payment of Replacement Fee. Any points validly earned using the lost or damaged Card shall be transferred to the new Card. PAC shall not be held liable for any points lost or redeemed as a result of damaged or lost Card.
5. Each Member is obligated to keep his/her information or personal data file up-to-date and correct by reporting any change or correction in name, address and other data. Members shall do so following the process stated in the Membership Application Form OR call PAC Customers Support Group (CSG) through the following hotlines: 526-2911, 524-0331, 526-3870, 524-0294, 0917-522-7797(Globe), 0917-790-3526 (Globe), 0998-862-7800 (Smart). Member understands and agrees that PAC shall not be liable for any loss, damage, injury or claim arising from his/her failure to keep updated or to correct his/her information or personal data file.
6. The Membership Card is not transferable in any circumstance, and is valid for use only by the Member who must present it when conducting any transaction. The Member may not use this Card to earn points from transactions of other customers.
7. The Membership Card may not be used at the time of purchase and/or activity that is identified by PAC as not eligible for earning PPC points.
8. PAC reserves the right, at its sole discretion and without prior notice, to cancel the Membership Card and all Awards and Privileges that go with said membership.
9. Membership will terminate automatically and all awards and privileges void upon the death of the Member.
1. Members, whose membership accounts are active, may redeem their accrued PPC Points in exchange for goods. These Points cannot be exchanged for cash.
2. Rules and points for conversion to peso value are described in the Members Application Form. Points earned are automatically convertible to equivalent peso value and may be redeemed after fourteen (14) working days. PAC reserves the right to change the value of earned Points at any time without need of prior notice.
3. Points earned may be used to pay for any product or any items that may include or exclude from time to time at its sole discretion without need of prior notice.
4. PPC Points is not transferable in any case. It is only for use of the Card Member who must present his/her Card in any transactions. Over-the-phone and walk-ins transactions are subject for verification
5. PAC reserves the right to make adjustments on earned or accrued points as it sees fit including, but not limited to, deduction of points arising from returned goods purchased or from disputes.
6. It is the responsibility of the Member to keep transaction receipts, which may be required in case of request for point’s verification by the Member.
7. For the redemption of PPC Points, those points reflected in PAC records shall be deemed correct and PAC has the exclusive right to determine the available amount of points to be or redeemed by the Member.
8. Redeemed Points are not exchangeable for other Points, or refundable, replaceable, or be sold or transferred for cash or credit, under any circumstances. Points redeemed may no longer be changed, revoked, or cancelled.
1. The provisions of these “Terms and Conditions” as well as all other rules and procedures issued by PAC from time to time in connection herewith may be cancelled, changed or modified at any time and for whatever reason at the sole option of PAC without need of prior notice.
2. PAC reserved the right to terminate the Program at any time at its sole discretion. In the event of the Program, all accrued Points earned in the Card will render invalid unless used within ninety (90) days after the Program’s official date of termination.
3. Any breach of these “Terms and Conditions” rules, regulations, policies, whether intentional or not, may result in termination of membership, forfeiture of Points and cancellation of all privileges as described above.
4. The Member hereby understands and agrees that the successful operation of PPC Rewards Program is based on the cooperative interaction among PAC, the Members and PPC Rewards Program Service Group.
